Patient Information and Responsibilities


Not all insurance companies cover annual physical exams and associated lab work. Before you have such an exam, labs, or procedures, please contact your insurance company to verify if the charges are covered under your plan. Benefits vary according to your particular plan with your insurance company.

All new patients should arrive 15 minutes prior to their scheduled appointment. Adequate time is needed for paperwork completion and insurance verification.

We make scheduled appointments for all visits. Walk-in or work-in patients will be accommodated according to the doctor's schedule.

For all established patients, we ask that you notify the front-office staff of any changes in address, phone numbers, or insurance coverage.

If you have an emergency after hours, weekends, or holidays, please dial (212) 873- 1840. We have our answering service to reach the physician on call. Please call 911 if you feel that you have a life-threatening emergency or problem that cannot wait for a callback.

Please notify our office two days in advance of the need for a prescription refill . This will allow sufficient time to complete the refill before running out of medication. Your pharmacy can always fax the request for a refill (212) 724 6158.

We ask you to contact us for results one week after tests; It is our policy to alert you to problems but your call help prevent delays.

We charge 75 cents per page of records.

For patients who need to obtain a referral to see a specialist, except in case of medical urgency, you need to allow between two to three days for a referral request to be processed and sent on to the specialist. If you choose to visit without a referral, you may be responsible for payment for that visit.

The Health Insurance Portability and Accountability Act of 1996 (HIPAA) has been enacted to protect the confidentiality of the patient's health information. Therefore, we will not be able to disclose any information regarding your care here at Crosspark Medical, PLLC to family members, friends, etc., without the patient's written consent. With this new regulation, we are limited as to the information that can be left on voice mails or answering machines.
